The
purpose of the Funding Source Identification Form is to provide the Budget
Office with the current year/budget year source of funding for new positions
and currently established positions that require a funding change due to a
reclassification, in-grade salary increase, time-base change, etc. The form will be available at: http://adminfin.csusb.edu/finance/budget_office.htm.
If using the form for a new position,
it must be submitted to the Human Resources Office along with the Position
Transaction Form. Any questions regarding
the Funding Source Identification Form should be directed to the Budget Office
at ext. 75134.
Steps
for completing the form are listed below.
New/Modify: Check the box according to
the reason for Funding Source Identification.
Check “New” for a new position. Check
“Modify” for any change to an existing position.
Effective Date:
Enter the starting date that funding is required.
Fraction: Enter the position fraction
(Example 1.0 or.5).
Position #: If the position is currently
established, enter the PeopleSoft position number assigned (Example: 00001234).
Position numbers for new positions will be assigned by the Budget Office
when the position is created.
Job Code: Enter the four digit job code
associated with the position description.
(Example: ASA, I = Job Code #1032)
Department: Enter the department name where the position
is located.
Range/Level: Enter the range or level of the position.
Incumbent: Enter current employee name,
or if this is a newly established position, enter “Vacant”.
Position Title: Enter the title of the position being affected. (Example: ASA II)
Partial Month
Cost:
If the new position or salary change is effective mid-month call the Budget
Office for instructions on calculating cost.
Increase in Monthly
Salary Cost: Enter the monthly salary difference for the reclassification, in-grade
salary increase, etc. For new positions,
enter the monthly salary.
Benefit %: Use the table at the bottom of the form to see
what the benefit percentage rate is for a new position.
Increase in Current
Year Salary Cost: Use the amount from the “Increase
in Monthly Salary cost” line and multiply it by the number of months needed
for the current fiscal year. If applicable,
add in the amount from the “Partial
Month Cost” line.
Current Year
Benefit Cost*: Do not enter calculated amount.
This line will be completed for you by a built-in formula.
Total Funding
Required for Current Year (Salary & Benefits)*: Do not enter calculated amount.
This line will be completed for you by a built-in formula. This
is the total amount required for transfer in the current fiscal year.
Funding Source(s):
From: Fill in the chartfield value to be used to cover the costs associated
with this position for the current fiscal year.
If using more than one funding source, enter the chartfield value for
each and the amount to be associated with each one. To: Fill
in the chartfield value where the position salary costs will be charged. The amount will be filled in for you by built-in
formula. The central benefits chartfield
will be completed by the Budget Office, you do not need to make any adjustments
to this line.
Section C: Budget Year Funding
Required (
Increase
in Monthly Salary Cost: Same as in Section B.
Benefit %*: You do not need to enter percentage amount, it will
be copied from Section B by built-in formula.
Increase in Annual
Salary Cost: Use the amount from the “Increase
in Monthly Salary Cost” line
and multiply it by the number of months the position draws salary in a fiscal
year. (Example: 10mo., 11mo., or 12mo.)
Annual Benefit
Cost*: Do not enter calculated amount. This line will be completed for you by a built-in
formula
Total Funding
Required Annually (Salary & Benefits)*: Do not enter calculated amount.
This line will be completed for you by a built in formula. This is the amount required for transfer on
a permanent basis.
Funding Source(s):
From: Fill in the chartfield value to be used to cover the costs associated
with this position on a permanent basis. If
using more than one funding source, enter the chartfield value for each and
the amount to be associated with each one. To: Fill
in the chartfield value where the position salary costs will be charged. The amount will be filled in for you by built-in
formula. The central benefits chartfield will be completed by the Budget Office,
you do not need to make any adjustments to this line.
Print the form and obtain the authorized
signature.
