Make-Up Time

All non-exempt employees must complete the Request for Make-Up Time form for each and every time they wish to request to make-up hours lost due to their own personal needs or obligations.

Procedure:

1. Obtain a Request for Make-Up Time form which is available on-line or at the Foundation Human Resources Department.  This form needs to be completed and approved by the employee’s supervisor prior to taking the time off.

2. Employees can only make-up the lost time in the same week, and he/she cannot work more than 11 hours in a single workday.

3. After the approval, the employee must attach the Request for Make-Up Time form to their timesheet or time card.
